How to start a diet correctly (or fix one that isn't working)

Low fat, low carb, low sugar, vegan, paleo, and on the list goes.
If you've been struggling to lose weight, many times it's simply because you're following every piece of advice you hear in hopes that the fat will instantly fall off.
"Just eat low carb!"
They say. That'll do the trick.
So you try eating low carb. Maybe you lose a few pounds, but eventually it stops working.
How do you set up a diet that doesn't stop working?
One that keeps the weight coming off as long as you follow the directions? A diet based on consistency, rather than the promise of magic pills?
